2020-2021 Athletic Physical Information

 

ONLY students new to athletics, marching band, or other activity requiring a Pre-Participation exam; and/or any student with a “Yes” answer on questions 1-6 on the Medical History form will be required to complete the physician portion of the physical.  

 

ALL students MUST complete the online forms in SportsWare (just like last year) and, the Medical History is included within that program this year.

 

If a student needs a Pre-Participation Examination from a physician, they may print the form from our website and take it with them to the physician.  PLEASE DO NOT use last year’s forms.  The forms this year contain changes from the UIL and CISD.

 

The only hard copy form that will be accepted is the Pre-participation Exam that has been completed by a physician.  All other forms must be completed online.

 

SportsWare:

 

If the student had an account last year (cleared to play), they DO NOT re-register.  The parent should log in with the email and password that they created last year; if they cannot remember, simply choose the “Forgot password” link.

 

Below are the instructions for new students as well as returning students for SportsWare, and also a digital copy of the Pre-Participation Exam form.  The district has posted the forms online and the links are below.  The link to SportsWare is www.swol123.net   If returning, please login using the Login box at the top right of the page.  If new to athletics, please “Join SportsWare” which is the third box on the right.
